What's Happening in Hamilton, ON…

Several years ago, Hamilton area hospitals undertook a pilot project designed to provide better patient outcomes, and the program was such a success that it recently received praise and was presented as a model that could be applied throughout the rest of the province.

The healthcare system that has been adopted in Hamilton has been adapted for patients undergoing hip or knee replacement surgery, heart and lung issues, and lung cancer surgery.

According to the Donna Johnston, who is the project lead, a team of carers helps each patient, with the goal of providing more comprehensive treatment at home. Each person is assigned a care coordinator, who makes sure they have the supports they need, and that they have access to help if they need it at any time.

At recent luncheon held at the Empire Club of Canada, the province’s Health Minister noted how much of a difference the program had made and urged other health care centres to create similar models of their own.
