Caulfield to host advance care planning talk

Alfred Health is holding an information day to explain what advance care planning is, and what it means for individuals and their families.

Advance care planning is designed to ensure that a person’s healthcare will be consistent with their beliefs, values and preferences even when the person is no longer able to express their wishes.

As a routine part of healthcare, advance care planning involves discussions between individuals, their healthcare professionals and their families. It creates a document that will be used to make decisions about a person’s treatment and care if the patient themselves is too sick to make or communicate their choices.

An Advance Care Directive can either spell out specific treatments that an individual wishes to refuse or consent to, or state the values and preferences that the individual wishes their medical treatment decision-maker to consider when making decisions about medical treatment on their behalf. The instructions in an Advanced Care Directive are legally binding and the document can be uploaded to an individual’s My Health Record.

Information sessions such as this often use brochure printing to create leaflets that attendees can take away and read later to help them understand the information that has been covered.

The information session will be held on Thursday October 10, from 10:30am to 12pm at Caulfield Hospital, which is at 260 Kooyong Road in Caulfield.
