Free immunization event in Ocoee

The West Oaks Mall in Ocoee will host a free Back-to-School Immunization event for children attending Orange County Public Schools. The event is hosted by the Florida Department of Health in Orange County.

Schools and businesses in Orange County could use printing services to create Posters advertising information on such events. Posters for this event could include, for example, a listing of documents that are needed to receive the vaccine. These documents consist of an original Birth Certificate, other government ID or a passport along with the most recent immunization record.

The state of Florida requires children be vaccinated to enroll in public schools or daycares. Those attending the event will receive the required immunizations and a certified DH80 form. Children must be accommodated by a parent or related family member with a valid ID. This means they must be accompanied by either a parent, grandparent, step-parent, sister, brother, aunt or uncle. Legal guardians are required to bring the court ordered paperwork. Non-guardians or non-related adults can bring children aged seventeen or younger providing they have a notarized letter from the parent or guardian.

Immunizations are provided on a first come first served basis and will be administered until the daily supplies are depleted so it is recommended that those needing the vaccines arrive earlier in the day.

The West Oaks Mall is located at 9401 W. Colonial Drive. The event will take place from Monday, July 22nd through Sunday, August 4th. Vaccines will be administered during the hours of 8:00 a.m. and 2:00 p.m. each day during that time.
