A Cary physicians’ network has teamed up with the leading patient intake platform in the country to offer a more personalized experience for patients.
The Community Care Physician Network (CCPN) in Cary will be using the automated registration, communications, and clinical applications of Phreesia to reduce the exposure between staff and patients during intake, and to address the currently unmet social needs of patients.
A pediatrician at Blue Ridge Pediatric and Adolescent Medicine in Boone, North Carolina and Co-President of CCPN, Dr. Greg Adams, said that Phreesia has created a product that enables patients to schedule their own visits and fill out screenings and questionnaires before their visits. He added that there are tools that allow staff to ensure a safe appointment for patients. Flyer printing is often used to provide more details about such products and their tools.
Phreesia can also be used by patients to identify social drivers of health, such as a lack of affordable housing and food insecurity. This is part of an initiative by the North Carolina Department of Health and Human Services.
Chaim Indig, CEO of Phreesia, expressed his pride in being able to partner with CCPN, an independent practice that is interested in quality improvements. He said it is now more important than ever to screen patients for unmet social needs and risks to their health.
