There is a school of thought that primary care physicians and their patients can work best when following a collaborative model of healthcare, and a Warwick event will be sharing ideas about how to make this happen.
The workshop will be split into two sessions, and the first will touch on a variety of topics that will help doctors and other healthcare providers to improve outcomes by providing team-based care and an increased level of patient engagement. The seminar will break for lunch, and in the afternoon, the participants will be led through an exploration of how using medical record software, and also the Rhode Island All Payer Claims Data Base, can both create reports and assist in utilizing different health plan supports.
The organizers noted that one of the themes for the day will be how monitoring population health, as well as increasing the levels of patient activation, can lead to improved healthcare outcomes. Those who attend can choose to be at either session or both, and anyone who works in the healthcare industry and feels this event may be helpful to them is welcome. Postcard printing can be used to create mailouts that let all local medical professionals know when an event like this will be taking place.
